In consideration of being allowed to participate in any way in South Dekalb Sports Association related events and activities, the undersigned acknowledges, appreciates, and agrees that:

1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist

2. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releases or others, and assume full responsibility for my participation

3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately

4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS South Dekalb Sports Association,  their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

PHOTO RELEASE FORM

I hereby authorize South DeKalb Sports Association to publish the photographs taken of me and any undersigned minor children, and our names, for use in the South DeKalb Tribe’s printed publications and website.

I release South DeKalb Sports Association from any expectation of confidentiality for the undersigned minor children and myself and attest that I am the parent or legal guardian of the children listed below and that I have the authority to authorize the South DeKalb Tribe to use their photographs and names.

I acknowledge that since participation in publications and websites produced by South DeKalb Sports Association is voluntary, neither the minor children nor I will receive financial compensation.

I further agree that participation in any publication and website produced by South DeKalb Sports Association confers no rights of ownership whatsoever. I release South DeKalb Sports Association, its contractors and its employees from liability for any claims by me or any third party in connection with my participation or the participation of the undersigned minor children.
